Management

Assisted Living Administrator

  • Responsible for financial stability of Assisted Living facility from both census development and expense control
  • Develop and maintain relationships with family members and residents
  • Oversee and manage Assisted Living Review Schedules, vacations, evaluations, hiring, firing, disciplining of staff
  • Ensure staff attends in-service meetings and has required training such as First Aid Record and prepare monthly statements
  • Review all bills with the Administrator
  • Maintain database for referrals for Assisted Living as necessary
  • Responsible for overseeing needed supplies and inventory for both dietary and housekeeping to ensure that costs remain in budget
  • Oversee and maintain cleaning/laundry schedule to ensure cleanliness of all areas (resident rooms and common areas)
  • Responsible for overseeing activity department
  • Assist in development of new programming and coordination of newsletter
  • Responsible for maintaining license and operation of beauty shop
  • Responsible for keeping all records up to date to comply with state regulations
  • Responsible for maintaining employee records in coordination facility business office and GMS home office
  • Responsible for tour and referral process
  • Responsible for overseeing receivables and deposits
  • Attend Accounts Receivable reviews
  • Responsible for overseeing mealtimes and dietary department at Assisted Living to ensure compliance
  • Responsible for overseeing and maintaining preventative maintenance

 

MDS Nurse

  • Review residents admitted and compare physician orders with each resident’s plan of care to assure appropriate implementation of the physician’s plan of care
  • Schedule routine MDS assessments (initial, quarterly, annual) to maintain compliance with regulatory time frames
  • Schedule ARD for all routine MDS assessments, following the most current RAI User Manual instructions, with input from the Interdisciplinary Team
  • Schedule change in status assessments as instructed
  • Complete Re-entry and Discharge tracking MDS within 7 days of the event
  • Communicate weekly team meeting date, time, and the following to team members: Resident name MDS assessment type (Initial Quarterly, Annual, Change in status, PPS) ARD RAPs due (as applies) Completion due date
  • Communicate schedule to resident and appropriate family members/legal guardian
  • Assist with and/or complete data entry onto the computerized MDS record as directed by the RN Coordinator
  • Print appropriate MDS/RAP notes prior to the care plan meeting
  • Obtain signatures on completed MDS Notify RN if difficulty obtaining signatures for appropriate follow up
  • File completed MDS in resident’s clinical record
  • Electronically submit completed MDS’ to the state on a weekly basis
  • Retrieve and maintain validation reports with each submission, discuss errors with the RN
  • Maintain current copies of informational updates on the state’s web site
  • Maintain current copies of the analytic and QI/QM reports
  • Distribute copies of informational updates, current QI/QM reports with administrator, DON, RN Coordinator, etc. as instructed
  • Review reports of timely completion of resident MDS assessments and care plans with recommendations of follow up action to the MDS Coordinator
  • Participate in patient care plan conferences
  • Complete reports and assignments accurately and adhere to established time schedules
  • Establish open lines of communication between all nursing personnel and consistently take follow-up action as necessary
  • Obtain the resident assessments from pertinent departments
  • Put information from all departments’ assessments into the MDS computer system and obtain MDS and RAPS as needed
  • Identify special nursing problems and emergency situations and initiate an immediate plan of action according to the nursing home policies and procedures
  • Assist in the admission/transfer and discharge documentation of residents according to federal and state guidelines
  • Follow all federal and state regulations regarding the currently approved MDS and keep updated as changes occur through continuing education
  • Act as a Charge Nurse and cover the floor/various shifts as needed for the best interest of the facility
  • Find and attend continuing education programs appropriate to this position
  • Maintain records of appropriate documentation according to federal, state and local regulations.

 

Admissions Nurse

  • Follows established standards of nursing practices and implements facility policies and procedures
  • Interpret existing policies and procedures to nursing assistants, restorative personnel, residents, families and physicians
  • Supervises and evaluates all direct resident care and initiates corrective action as necessary
  • Make frequent rounds in the facility monitoring meal time and assisting in Dining Room and on the halls as necessary
  • Function as a direct charge nurse and/or STNA at times and or as needed
  • Identify resident problems and emergency situations and initiates an immediate plan of action according to nursing home policies and procedures
  • Follow, supervise and evaluate the implementation of the Resident’s Right Program
  • Assist with evening admissions in collaboration with the charge nurses in the facility
  • Provide clinical supervision of nursing assistants and charge nurses responsible for providing direct resident care
  • Assist with covering staffing and making calls to cover any call offs Counsel nursing assistants and recommend and or initiate disciplinary action, when necessary, with follow up to the DON
  • Participate in the employee evaluation process by providing information on the clinical performance of nursing assistants to the DON or designee
  • Complete report for facility committees as assigned, active member of the Interdisciplinary Team Administer and document comprehensive assessment of each resident as needed and or as assigned
  • Assist with the development of an accurate and comprehensive care plan based on the resident’s needs
  • Notify physician, when necessary, of changes in residents’ conditions, weight, medication errors, medication not available, abnormal lab/x-ray results and any other unusual observations
  • Document resident care provided and resident’s response or lack of response to care provided
  • Personally obtain continuing education relating to the long term care field of nursing
  • Supervise response to residents’ calls for assistance
  • Follow facility discharge/transfer procedure and coordinates discharge/transfer with other departments, physicians and ancillary service providers
  • Possesses clear written and verbal communication skills
  • Act as a Charge Nurse and cover the floor/various shifts as needed for the best interest of the facility.

 

Treatment (Wound) Nurse

  • Obtain report and implement appropriate follow-up action as necessary in relation to the resident’s treatment needs
  • Conduct resident rounds as assigned to assess and report the skin condition of each resident, and take necessary follow-up action
  • Identify special nursing problems and emergency situations and initiate an immediate plan of action according to nursing home policies and procedures
  • Instruct nursing assistants in proper, preventive safety measures and use of adaptive equipment to meet the needs of each resident
  • Control supplies and equipment within the unit
  • Document resident care provided and resident’s response or lack of response to treatment provided
  • Assess and report changes in resident’s condition to physician, nursing supervisor and responsible party and to take follow up action as necessary
  • Assess special skin problems weekly as to stage, size, sites, depth, color, drainage, and odor of condition
  • Perform basic nursing skills
  • Obtain physician’s orders and any revisions for treatments needed
  • Review and update resident care plans as necessary and as assigned in compliance with regulations
  • Participate in resident care conferences
  • Provide direct resident care
  • Give daily instructions to nursing assistants regarding care that relates to treatments
  • Compile information for facility committees as instructed
  • Administer treatments according to physician’s orders, to accurately record treatment performed, and to keep weekly progress notes
  • Provide clinical supervision to nursing assistants
  • Assist in the admission/transfer and discharge of residents as necessary and as assigned by the DON
  • Complete admission/transfer and discharge documentation

Housekeeping Director

  • Purchase housekeeping and laundry supplies
  • Supervise receiving and storage of linens and supplies
  • Represent the Housekeeping/Laundry Departments
  • Participate in employee selection, evaluation and termination
  • Responsible for communicating pertinent information to employees
  • Prepare documentation and cleaning schedules
  • Plan and conduct in-service training programs
  • Assume responsibility for high sanitation standards
  • Ensure proper maintenance and repair of equipment
  • Perform cleaning procedures in accordance with facility policy
  • Observe waste disposal procedures in accordance with facility policy
  • Assure proper care and use of housekeeping/laundry equipment
  • Report any observed area in poor repair to the Administrator and to the maintenance department
  • Dispense soap, paper and other housekeeping supplies
  • Assist with the implementation of OSHA regulations as they pertain to housekeeping/laundry departments
  • Operate an automated laundry solution dispensing system
  • Prevent the spread of infection through proper handling, storage, washing and transporting of all garments and linens
  • Keep MSDS book updated with all new chemicals of housekeeping and laundry departments
  • Prevent spread of infection through proper handling, storage, washing and transporting of all garments and linens
  • Performs daily rounds to ensure a healthy, clean safe environment is maintained at all times
  • Ensures all hallways and entrances are uncongested at all times
  • Responsible for ensuring proper safety measures are used when housekeeping/laundry duties are being performed (e.g., wet floor signs, chemicals are secure and labeled, etc.)
  • Supervise proper labeling and handling of all resident’s clothing
  • Work with all corporate Supervisors to mainstream policies and procedures
  • Perform, oversee and schedule all floor care procedures
  • Ensures all state and federal guidelines are met
  • Assist Office Manager with housekeeping and laundry payables
  • Attend in-services and seminars as directed by the administrator of you facility
  • Perform all housekeeping and laundry positions
  • Perform other related duties as directed by the Administrator.

 

Other Departments

Dining Services Server

  • Be responsible for setting up resident trays and dining room tables
  • Be responsible for meal service including tray service, servery kitchen or dining rooms
  • Be responsible for bussing tables in the dining room
  • Be responsible for assisting in preparing, portioning, and covering foods and beverages
  • Be responsible for a clean work environment
  • Assist in the proper care, use and cleaning of food service equipment
  • Be responsible for all aspects of dish washing including scraping soiled dishes, placing them in the dish machine, operating the automated dish machine, supervising travel through the dish machine, sorting clean dishes and transporting them to storage areas.

 

Dining Services Aide

  • Responsible for setting up resident trays and dining room tables

Dining Services Cook

  • Responsible for safety handling and preparing appetizing and palatable food
  • Follows written menus and recipes and can understand and follow menu extensions
  • Understand therapeutic diets and serves them as per physician’s orders
  • Follows menus and recipes avoiding food waste
  • Serves food according to meal schedules
  • Responsible for care and cleaning of food service equipment.

 

Housekeeping Aide

  • Complete daily cleaning schedule in accordance with facility policy
  • Follow and perform infection control procedures
  • Ensure all safety measures are used when housekeeping duties are being performed (e.g. wet floor signs, chemicals are secured and labeled, etc.)
  • Report/Restock any supplies that need to be replenished
  • Assist in the proper care and use of housekeeping equipment
  • Identify and reports any equipment malfunction to supervisor
  • Report all unsafe conditions, defective equipment, and maintenance repairs to supervisor
  • Dispense soap, paper and other housekeeping supplies
  • Attend in-service training programs for housekeeping employees
  • Observe infection control procedures related to the Housekeeping Department
  • Follow OSHA regulations as they pertain to the Housekeeping Department
  • Assist with laundry duties.

 

Laundry Aide

  • Maintain a steady work pace to produce an adequate supply of clean garments and linens
  • Place garments and linens in drum of electrically powered washer
  • Identify and report any malfunctions in laundry equipment
  • Prevent the spread of infection through proper handling, storage, washing and transporting of all garments and linens
  • Add soap and other solutions to laundry
  • Operate an automated laundry solution dispensing system
  • Transfer clothes from the washer to the dryer
  • Hang, fold and stack garments and linens
  • Report any garments or linens in ill repair to supervisor
  • Distribute clothes to residents’ rooms
  • Observe infection control procedures as they pertain to the Laundry Department
  • Sort clothing as directed
  • Clean and maintain the laundry room in an orderly fashion
  • Follow OSHA regulations as they pertain to the Laundry Department
  • Attend in-service training programs for laundry employees
  • Notify supervisor of supplies and linens needed
  • Assist with housekeeping duties.

 

Life Enrichment Aide

  • Facilitate group programs as assigned in a timely and efficient manner
  • Provide individual programming as scheduled
  • Provide materials/supplies necessary for successful independent programming
  • Complete required documentation as assigned
  • Be flexible and creative to reprogram an unsuccessful program or one that has been cancelled
  • Encourage residents to attend activities provided, but respect the right of each resident to decline
  • Attend and contribute to recreation department meetings
  • Provide a safe environment by keeping recreational areas free of debris
  • Assist in transporting residents to and from activities
  • Follow instructions and evaluate results
  • Understand, organize and complete tasks assigned by supervisor despite interruptions
  • Maintain recreation department equipment and supplies
  • Work with volunteers as assigned by recreation director, assisting in the recruitment, orientation, training, communicating and recognition of volunteers
  • Assist with planning and leadership of recreation programs
  • Promote facility programs to residents, staff, family and volunteers
  • Comply with state and federal regulations and facility policies regarding recreation programs
  • Observe infection control procedures related to the recreation program
  • Follow OSHA regulations as they pertain to the Activities Department.

 

Receptionist

  • Maintain accurate emergency telephone number list

  • Voucher building invoices for payment as needed
  • Operate the Resident Funds Management System and maintain an accurately balanced account
  • Maintain accurate medical staff telephone number list
  • Operate facility paging system
  • Operate the facility computerized business programs
  • Communicate effectively on telephone, via e-mail, in person
  • Receive, sort and distribute mail
  • Neat, clean and appropriately dressed to present an appearance acceptable to residents and to the public
  • Handle emergency situations in a calm, effective manner
  • Screen all incoming calls
  • Handle all correspondence as assigned
  • Interact with residents and resident representatives and direct them to appropriate individuals
  • Screen all individuals coming into the facility and exercise good judgment in granting entrance
  • Accumulate and maintain accurate statistics as required and directed
